Abstract

Zusammenfassung. In den letzten Jahren haben sich viele antidiabetische Medikamente unterschiedlicher Substanzgruppen auf dem Markt etabliert, die jeweils verschiedene Wirkmechanismen und Nebenwirkungen aufweisen. Dazu kommt, dass seit 2012 die Richtlinien dahingehend verändert wurden, dass nicht mehr ein starres Stufenschema die Abfolge der Antidiabetika vorgibt, sondern dass aufgrund von individuellen Kriterien bei jedem Patienten die Therapie «massgeschneidert» werden soll. Dazu stehen theoretisch nach Metformin alle Substanzklassen allein oder in Kombination zur Verfügung. Alle Therapieansätze werden in Lifestyle Veränderungen wie gesunde Ernährung, körperliche Aktivität und Gewichtskontrolle eingebettet. Diese individualisierte Therapie gelingt am besten, wenn dem Praktiker nach der Festlegung des HbA1c Zielwertes die jeweiligen Vor- und Nachteile der einzelnen Medikamente bekannt sind und sie so nach klinischen und patientenspezifischen Gesichtspunkten verabreicht und kombiniert werden können.

Several antidiabetic agents from different substance classes have entered the market during the last few years. These drugs have different mechanisms of action and adverse effects. Added to this is the fact that since 2012 the guidelines have adopted a more individualized approach compared to the former rigid, step-wise guidelines for the management of type 2 diabetes. According to new guidelines, each patient should receive a tailored treatment strategy. In theory this means that after establishing treatment with metformin, all substance classes alone or in combination are possible. All these therapeutic approaches should occur in the setting of lifestyle modification such as healthy eating, physical activity and weight management. The success of this approach depends on the practitioner knowing which treatment options are available as well as their advantages and disadvantages so they can be applied in the most appropriate manner once the personalized HbA1c target value for a given patient has been established.
